Efron spent his teens and early twenties as a slight, cast-as-the-heartthrob actor — High School Musical (2006) and Hairspray (2007) both show a lean build with no real muscle. We couldn’t find a sourced weight for that era, so it’s left out of the table below rather than guessed.
The picture is on the record even where the weight isn’t. A publicity image of Efron and Corbin Bleu from High School Musical, dated 8 February 2006 is the baseline this page is arguing from: eight years before Neighbors, at 18.
The first physique with real numbers behind it is Neighbors (2014): reportedly around 155–160 lb (roughly 72.6 kg) at about 10% body fat, per fitness-press coverage of his training. Three years later, for Baywatch (2017), reports put him 5–10 lb (2.3–4.5 kg) heavier — call it 170 lb (77 kg) — while some outlets claimed as low as 5% body fat for the shoot. Efron’s own account complicates that number: in a Men’s Health cover story, he said the look “required Lasix, powerful diuretics” and that there was “too little water in the skin” — meaning some of that shredded look was dehydration, not fat loss, and the widely quoted 5% figure is likely softer than it sounds. He also said the regimen gave him insomnia and “a pretty bad depression” that took six months to lift after filming wrapped.
The bigger jump comes later. For The Iron Claw (2023), playing wrestler Kevin Von Erich at age 35–36, reports put his gain at roughly 15 lb (6.8 kg) of muscle over about six to seven months, landing near 185 lb (84 kg) at an estimated 9% body fat — his biggest frame on record, bigger than either Neighbors or Baywatch. Run the fat-free mass through the site’s FFMI formula and Iron Claw is also his leanest-biggest era by the math: FFMI around 25.2, just over the Kouri et al. drug-free ceiling of 25.0. Baywatch, despite the famous six-pack, computes lower (~23.7) because there was simply less muscle under the leanness.
That rate of gain is the real flag. Natural lean-mass ceilings for an experienced lifter — and by 2023 Efron had been training seriously for at least a decade — run about 2–4.5 lb (1–2 kg) per year. Adding 15 lb (6.8 kg) of muscle in six to seven months is several times that ceiling, at an age (mid-thirties) where several other Hollywood transformations on this site also cluster. None of that is proof; it’s the kind of arithmetic that invites the question, not an answer to it.
On the record, Efron has joked about the topic rather than denied it outright. Asked about performance-enhancing drugs while training alongside Jeremy Allen White for The Iron Claw, he said, “Do I slip something in here?” — played as a bit, after White recounted the production telling them wrestlers were “on their own.” No admission, no failed test, nothing official.
Verdict: likely-enhanced — an opinion based on the math above, not a claim of fact. A borderline-natural FFMI on its own would be unremarkable; paired with a documented muscle-gain rate several times the natural ceiling for his training age, in his mid-thirties, it tips the read past “uncertain.”
Two caveats belong with that. The baseline is now anchored, loosely: press profiles put pre-Baywatch Efron around 155-160 lbs (70.5-72.5 kg) — his lean High School Musical-through-Neighbors range — and his Baywatch trainer’s camp claimed about 10 lbs of lean muscle added in a reported 12 weeks, holding under 10% body fat year-round. Ten pounds in twelve weeks on a lean mid-20s frame is aggressive but inside a well-coached natural window, which keeps Baywatch itself off the enhancement scale — it’s the LATER Iron Claw jump, at 35, that carries this page’s verdict. These early figures are press numbers, not weigh-ins, so the rate-of-gain math still rests on soft points. And by his own account the Baywatch look was a manufactured peak rather than a steady state: he has said it “required Lasix, powerful diuretics” and left him with bad depression after filming wrapped.
